I know I win the bad mom award for this one, but I have misplaced my daughter's birth certificate. What is needed to get her a new one? She's 4, so obviously has no photo ID or mail with her name on it. If my information sufficient? Also, she was born at Children's Hospital. Where do I go to get her a new certificate? Thanks!
